Form not allowing people to complete Contact and Address Fields

    I am stumped. I created a form that is not letting anyone fill in the fields for name, address, email and phone number. Can you please help.

    Thanks for reaching out to Jotform Support. The form you shared has been deleted. Can you provide the URL of the form that's not letting users fill some fields? This guide will show you how to do that. Also make sure that 'Read Only' is not turned-on for those fields:
