Good Morning!
I have an approval flow created in the Property Improvement Permit form. I would like to send a copy of the completed form with the form "certificate" attached back to the original applicant at the end of the flow. When I try to do this, it says the form is already in use in another approval flow.
I have attached a link to what the workflow looks like, and a pdf showing what I would like the workflow to do.
I appreciate any insight you might provide - BS

Hi assistantmp,
Thanks for reaching out to Jotform Support. Sorry if you're having difficulties with your approval flow. I checked your approval flow but I can see the form connected. I understand that you want the second approver to fill out the certificate and send it to the applicant. Instead of adding the same form to the last element, you can use the Email element and attach the PDF form from the Main and second forms. Check out the screenshot below:
That is because you cannot use the form twice on the same approval flow. So instead, send them a copy of the main form they filled out and the certificate form filled out by your team member. See the screencast below on how to do that:
Give it a try and let us know if you have any other questions.

This link is to my approval flow--
Here is what I want to happen:
form comes in, assistant verifies payment & approves
form goes to jcanty for review; upon approval, he fills out the certificate, and upon approval the filled certificate gets included in the email to the applicant.
I have tried SEVERAL VARIANTS of placements of items in the flow, and can't seem to get this result.
I appreciate any insight you might offer! - BS

Hello assistantmp,
Thanks for getting back to us. Unfortunately, the filled certificate by jcanty cannot be attached in the Email element in your Approval Workflow as this is a different form. You might want to send the certificate directly in the Certificate Form to the email of the applicant. You can do this by adding the email address of the applicant in the Certificate Form and create a different Email Notification for the applicant to be received with the attached certificate.
Reach out again if you have any other questions.

The certificate will be posted at the jobsite; I would like for the recipient email NOT to be visible on this certificate; is that possible? - BS

Hi assistantmp,
Thanks for getting back to us. According to my understanding, you want to send the Certificate to the first form submitter. If so, copy the applicant's email address from the first form and paste it into the second form:
Then, on the Certification form, you need to create Email Notification to sent filled form to the applicant email to be a recipient:

So I have to create a clone of the main form each time one comes in to process? Not sure how creating a new form and copy/pasting from another form anything other than more work than doing this on paper :(
I am struggling to understand. SORRY!

Hello assistantmp,
Thanks for getting back to us. You do not need to create a form when following my colleague's suggestion. Kindly do the suggestion on the Form Element below:
