I want to assign a numeric value to each of the 5 answers for each question and then provide the user a total score at the end of the survey. It looks like Jot Form will do this, but I need some guidance for developing the form.

Yes we have this available. However, this is only available on radio buttons, check boxes and dropdown tools. To assign calculation value on these fields, follow these steps:
1. Assuming you have a radio field for the answers to be chosen in your form:2. Right click the field and open its properties.3. On the properties wizard, set the calculation value like the following.
