Display Textarea when a Check Box is selected

2) the 3rd question (fund Choice) - I was hoping to use a combined check box list and free text list so clients could tick the desired funds then write next to them the percentage of their salary sacrifice they wish to invest in each fund. Is this possible as I could not see how to do it on the form?
