Help creating multi-page form

Hello,

I'm trying to create what I believe should be a very simple 5 question form, however I'm having difficulty figuring out how to customize it to my preferences.

Here are the specifics:

I have a limited amount of space on my website to have this form so I need each question to appear ONLY after the preceding question has been answered.

Here is an example of how the form would be:

Question 1 (visitor types information into text box and clicks "next")

After they click "Next" that question disappears and Question 2 pops up in the same place as Question 1. They complete Question 2 and type "Next" and so on.  Once they have completed all 5 questions they click "submit" and the results of the form are emailed to me.

Please help me understand the best way to go about creating this form.  Thank you in advance for your help!
