How to have form contents go into a new form based on an approval or denial process?

I asked you guys this question before and you said I couldn't do it the "logical way".

Essentially what I need is a form that needs to go to a supervisor, the supervisor then needs to check a box for approval or denial of the request (for example, a time off request). If the supervisor denies the request, it should email the employee back and tell them they've been denied. If the supervisor approves it, it needs to get sent to the lead manager for final approval, or denial (starting the process over again).

I had setup some rules that say if the approval box is checked send here, or there, but your developer said that email responses being sent out never work after the initial submission- so that process and logic will not work.

Has that changed yet? this was a couple months ago.

If it still doesn't send out emails after the initial submission- is it possible to have the contents of the submission sent to a new form, that can then do an email submission to the lead manager etc? It was a suggestion from you guys before...but now I really need this working.

    Thanks
