The form as working great!
Now it has disappeared after just a few days on the page.I am using form as a trial to see how your product works before considering upgrade options.
Along with this submittal, I've submitted the URL of the page where form belongs.

Hi,
I checked the page you referenced and I found this form in your page, available and working:

I checked your account to make sure you weren't over your submission limits, just in case that was the reason your form wasn't showing up, and you are not even close. From what I can tell, the form is embedded and ready to use.
If you are still unable to see your form in your page, let us know what browser you are using so we can check to make sure it is not a browser issue.
