I can't get my emails to work, even when i select the "Test Email" button. It shows the right email address but I'm not getting any emails. Can someone help? My contact information is 512-422-9988 or I can be emailed at sstephens@castlighthealth.com.

Hello Scott,
Upon checking your form's notification email alert, I did not find any issue with that. You have done the notification setup correctly. You should be receiving the submission emails correctly.
When I checked our email logs, I found that our email server was not able to send the submission emails. Sending of all the submission emails actually failed, please check the logs below:

You seems to have added Custom Sender Email in your account and I suspect that most likely the SMTP setting of have provided is not correct.
I would suggest you to delete the custom sender email you have added in your account and add it again. The following guide should help you: https://www.jotform.com/help/241-How-to-Edit-Delete-Sender-Emails-on-SMTP-Settings
Alternatively, you can just select "noreply@jotform.com" or "noreply@formresponse.com" in the "Sender Email" of your notification email alert. Please check the screenshot below:
