    How can I make the form warnings take effect on my form? I need to make sure certain fields are being filled out.

    To set your fields as required, you need to do the following steps:

    1. Edit your form
    2. Select the field you wish to be required to be filled up
    3. On the tool bar (on top of your form), click required

    4. Repeat step 2 for all the other fields you wish to be required
    5. Save your form

    In addition to this, to ensure that everyone fills it up, you can also add a noscript on your form so that forms with javascript disabled on their browsers will be compelled to activate it before visiting filling up your form:
