Our Jots forms have disappeared! We need them for physical therapists to refer children

    Hi!

     

    We are a nonprofit dedicated to improving the lives of children with physical disabilities.  We come to the rescue when insurance and Medicaid deny equipment and home modifications that our kids need.  We are not funded by the government. We were using Jots forms for referrals and for contact info.  Physical therapists and families could easily fill out the info that we need to get started on a new case.  The Refer a Child form has disappeared along with our Contact us form.  We desperately need them back.  Are you able to help?

    I see that your account is overlimit due to the storage space and hence the forms are disabled. Free accounts are allowed upto store only 100MB of files. You are required to clear some storage space so that the forms shall be enabled. I'm activating your account to help you to login into your account.

    Please login into your account and check the submissions that are occupying more space. Please make sure that you take a backup of your submissions before you delete because once the submissions are deleted they cannot be recovered in anyway.
