I will be using my form as an embedded form.
I do not want it accessible from the public.
I also do not want reference to jotform in url
Can you please provide?

Hi,
Though there is not currently a way to completely strip any reference to JotForm from your URL, you can create a custom URL for your form:
http://www.jotform.com/help/261-How-to-create-a-Custom-URL-of-a-form
Though the beginning of your link will still contain jotform. If you don't want your form accessible to the public, you would need to hide the embedded version behind a password protected page. You can also add a code to your form to show the contents:
http://www.jotform.com/answers/290633-Password-Protect-form
Though it does not exactly password protect your form.
