Why is my form still showing as disabled after I have enabled it?

When I select to disable a form, it disables, but when I enable it again it never changes the status of the form to enabled. I have a from named Switch my Style and the status is enabled but on my website http://www.glamfab2x2.com/pages/switchmystyle its still disabled. Please help. 

I checked both your website and your jotform in your account to take a look at your disabled form problem. However, when looking into it I was unable to find any such problem since both are showing that they are enabled on my end.

If you're still having an issue with it on your end then please clear your browser cache.

Your JotForm Account: 

- If it was disabled it would be grey and not black as it is now.

Your Website: 

- If it was disabled here you wouldn't see your jotform, only a disabled message.
