Adding my jotform to my GoDaddy website

I'm trying to put this on my website on GoDaddy. I used the simple set up for my website, and this won't show up as a page. Just numbers.

I believe based on what you have said, that you have used the <embed> option which is actually js embed option and shows you text instead of HTML elements.

What I would suggest is to use the iframe embed option instead.
