I see the submission failed to arrive, there is a problem with the SMTP of your email service provider. Please contact with your email provider and request them to add JotMail's IP Addresses to their whitelist.
If the problem still persist, you can use Mandrill to Send Emails From Your Own Email Address.
Meanwhile, you can try setting up another notification using "email@example.com" as the Sender E-mail. Hope this helps you, let us know if you need more help.
