I am trying to access my account "norcalgmat" with the email firstname.lastname@example.org and I am unable to do so. I have tried to reset my password and that is not working either. I need to cancel my payment/account as I no longer work for the company to which the account was tied. Can someone please help me with this?
My phone number is also 510 282 7820. I will no longer have access to the email above after friday. My personal email is email@example.com

As requested, we have cancelled your Premium JotForm account and you will no longer be billed in the future for this subscription. Your account will be downgraded to a Free account effective 2011-10-23.
