A potential customer let me know that my email form wasn't working, so I tested it out and it says that the form passed it's allocated quota and can't be used. I tried to log into my jotform account but couldn't seem to remember the password so I requested a password reset but I'm not getting that either. I have checked my spam folder as well and nothing there.
I checked to see if anything went wrong with the Paypal payments and for some reason, even though I signed on for a monthly subscription I only see the first payment on December 9th, but nothing this month.
Please help me out to get this working again, I really would like to keep the form as it is on my website.

Upon checking your web page where you have embedded your form, I found that you only have a guest account. This type of accounts are created if you do not sign up / sign in to your account and start building forms.
Please be noted that guest account has submission limit of only 20 submissions. Since you have already received 21 submissions, you see the form over quota error message.
To solve this problem, I would suggest you to sign up for a free account using your email address firstname.lastname@example.org and all your form will automatically move to your new account. Do get back to us with your new account username if you do not see your form, we will manually move it.
