Why do the SUBMIT, CLEAR, and PRINT Buttons not work on my form?

    The URL of my form is http://www.jotform.com/?formID=12445905102.  I have added a PRINT, CLEAR, and SUBMIT button at the bottom of the form and I cannot get any of the three buttons to work.  Can you help me determine what I am doing wrong and why the buttons do not work?  I believe I have the form laid out correctly but I am new at using JotForm and am not savvy when it comes to programming so there could be something in the programming that I've done that is prohibiting these three buttons from working.  I know I set up rules for when emails should be sent and to whom they should be sent (I found that information in another forum) but I am completely, stumped on why I cannot get the form to submit, clear, or print.  Any help would be greatly appreciated.

    http://www.jotform.com/form/12445905102 is the URL of your form.

     

    Its working fine in my tests on Chrome & Firefox.

     

    What browser are you using?

    Which version of Safari are you using? Two of those buttons (Clear Form and Print Form) are working okay from my end on Safari 5.1. and I'm betting that Submit Form would also work if I tried it.

    Are you perchance testing them from the form builder? (I noticed the URL you provided).
