When customers complete the online application form and submit, I am not receiving notifications, via my email?

Hi,
Upon checking the form embedded in your website, I believe you are not receiving email notifications because you are missing important fields in your "Emails" setup.
Here's a screenshot of the setting of your form. You are missing the "Sender E-mail" field.
You may also want to check this guide to prevent future email problems: http://www.jotform.com/help/208-How-to-setup-email-alerts-to-prevent-email-bouncing-related-issues.
Also, after setting up all the fields correctly please do try making a test submission by filling out your form and clicking submit, see if you can receive email notifications.
