This form is cut off in the middle

    Click on check or credit card and it'll show you the fields to fill out, but you can only see half of the field.

    Fix?

    This form is using iFrames for the options. In order to solve this we need to add/increase the width of the iFrames:

    Add the width of 600px using the method described above.

    Even link of the form you provided cuts off the hover text. When I go into the file, the width isn't even there - when I insert 'width:100%' it keeps cutting off the hover text.

    Have you checked if this issue with the form is still going on? I am checking your form (in Chrome, FireFox, and IE) and it is not cut off:
