Thanks for this. At this stage:
In the Form Picker dialog, choose your form, then select Continue in the
lower-right corner.
If I choose my form, will my form disappear or will I still have my form AND the quiz will be made? -

Hello Aleiya,
Thanks for reaching out to Jotform support. Your form won't disappear, what's going to happen is that your form will have the characteristic of the options you set on the QuizForm.
Give it a try and let us know if you have more questions.

This answers my question somewhat.
The question was prompted because I had done a regular survey, then thought to put a short quiz on the last page. Then I wanted it graded. I learned about the QuizForm and worried that if on the QuizForm I erased the 'survey' questions, then on the regular survey they would disappear as well.
But I just cloned the original survey and left just the quiz page and used that clone to make the graded quiz. Then I placed the link to the quiz in the thank you page of the regular survey.
It is an interesting set up that you can't see your quiz listed but have to enter the survey to get to it.
