Can you answer the questions below and possibly provide a demo of your product?

    1.     Do your forms have the ability to utilize conditional logic?  If so, to what extent?

Are approval workflows possible?  If so, can you provide details?

Do your forms have the ability to use email logic routing to return survey results to specific persons/admins?

    4.     Are the forms deliverable through both a mobile app and desktop (email, web link, QR code, etc.)?

    5.     When creating a list for a drop down menu, can I import a saved list or new list from an Excel or CSV file?

    6.     Is there a pricing structure that will allow sub-accounts?

    7.     Can the look of the form be customized (color schemes, graphics, etc.)?  If so, to what extent?

    8.     What type of reporting do you offer?

Once a form is created, can it be saved as a PDF form that can then be emailed as an attachment for users to complete?

Yes, you can set various conditions on your jotform to help you make it better for your visitors, or to allow you to add extra functionality or alter the current one.

There is no limit to the number of conditions that you can add and would depend on the actual conditions how much you can add them.

I would however like to point out that form builders are made for simplicity of use and as such they are not speed optimized in the same way a custom solution would be.

Since we can provide answer to only one question / issue per thread, I have moved your other questions to new threads.
