Two of my questions on my form are not working. You can enter text but when you receive the form via email after submitting the text does not show up.

Upon taking a closer look at your form's notification email alert template, I did not find any issue but the subject line does not seems to be correct. It has incorrect form fields name. Please check the screenshot below:
I have taken care of this issue in your email alert template. Now the submission emails you will receive will display the name and expo details correctly in subject line.
I would suggest you to test your form again and get back to us if the issue persists.
