The form I imported from FormsCentral looks right when going to it, but different in the submissions panel

The questions that require a yes/no response aren't shown when I print returned form.  Is there a way to fix this? The form is in a different format when it comes back. This just won't work like this. It also still says "adobeforms" in the URL but it seems to submit response to jotform just fine??

I took a look at your jotform here: http://form.jotform.us/form/50346476233151 and I see what you mean.

I do apologize for this and I have made a quick change to the jotform which you can see and clone from here: http://form.jotformpro.com/form/50355494423960

It should resolve the issue where you do not see the label of your questions, but I will still raise this to our developers so that they can take a look at it and modify our tools accordingly.
