In regards to my above comment. It's only an issue using Google Chrome. It is not a n issue with Internet Explorer or Mozilla Firefox.

If I understand correctly, the configurable list widget does not load fully in Chrome? Occasionally this widget does indeed have issues loading in some versions of browsers. Usually updating the browsers or simply refreshing the page does allow for the widget to show up. I just checked a test form in Chrome and the widget did show without issue.
If this does not allow for the widget to show up, let us know which version of Chrome you are using and we will be happy to have another look.

I cleared my cache and history yesterday and Chrome worked fine after that.
