How can I create a survey to select items in order of preference? e.g. Out of 10 listed items, select 1st, 2nd and 3rd most important?

Perhaps the Grading Widget will work on your requirement

Actually, there is also an Orderable List Widget which I think will work for you also.

Hey Jonathon - that's great. Thank you. Is there a form like these where I can add and extra 'blank' field for someone to suggest something for the list that I may have forgotten of not thought of?

    Hi,

You can clone the demo forms e.g. Orderable List Widget demo http://form.jotform.co/form/33392842334859. And then you can make changes on the cloned form. Would a text box tool sufficient to ask user input for the list you may have forgotten? If you're referring to another widget where user can add options on the defined list, these widgets may be of help. http://widgets.jotform.com/search/contribute
