Why are there empty columns between fields? Please advise what I should do differently.

I'm not sure how these empty spaces appear in the form, but it is possibly because you have pasted an HTML content to our Free Text HTML field from an outside source. The HTML consists of a few div elements and classes, take a look at the following screenshot:
I've cloned your form and I don't see it in the clone version. Here's my clone form: http://www.jotformpro.com/form/50452892173962.
I would suggest editing the Free Text HTML field and remove these classes/elements and use the Free Text HTML formatting only. If that won't fix the problem, then manually remove the empty spaces. Just click on it and click "X" button to delete (assuming that you're in the form builder):

I have done both. The empty columns are still there in the submissions.
Please change the Volunteer Request Form, Usher Request Form and Infraction Form to not have empty columns in the submission.
FYI: These forms were imported from Adobe Forms Central.

Actually the Usher Infraction Form is beautiful. The submissions do not have empty columns in them.

If you have done editing the form and removing excess fields that represents a blank column, you can either import the data again using Method 2 here: http://www.jotform.com/help/309-How-to-Migrate-Form-Data-from-Adobe-FormsCentral-to-JotForm.
Or you can just edit the display of columns in your submissions page.
I'm also checking your forms and can't seem to find the empty columns, could you provide the form URL so that I can check their submissions?
