I am using a form with PayPal integration. The form is here http://www.reelactorsstudio.com/#!registration/c21co. The issue that I'm having is when trying to fill out the form on a mobile device the zip code box goes missing and the person is not able to submit the document. Please help.

I checked your form on a Galaxy S4 and I was able to fill out every field. The zip code box was above the zip code label but it was available.
Unfortunately the template you used for your form was very specific to the contact form it was created form and may be throwing off the added fields a bit. It is not currently possible to adjust the CSS for this template in the form builder. Though I was able to fill out the whole form on mobile.
