how do I limit responses to one using a checkbox?

I'm creating a survey and can't upload the survey widget. Within survey tools my only rating options are star and best/worse. 

I've set up a check box that offers five answers (strongly agree, agree, etc.) but want to limit the response to one of the five in each checkbox.

I see that you are using checkboxes for some of the questions. I think you should be using radio buttons instead of checkboxes since checkboxes are intended to use with multiple selections and radio buttons are limited to one selection.

However, you can still limit the checkbox to select a maximum of one checkbox from the field properties.

Once the Maximum Selections is set as 1, it displays an error when multiple options are checked.
