Our apologies! We have an issue a while ago with regards to our server load, but everything has been fixed already. Right now, I was able to load the page and the form showed up quickly. Kindly re-check and update this thread if the issue persists to your end.
I would also suggest clearing your browser's cache. Clearing your forms cache will also help. Here's how to clear your forms cache: http://www.jotform.com/help/229-How-to-Clear-your-Form-Cache.
