Create an order form that allows users to type their Personal Design and Add Extra Fee

Good Afternoon,

Is there a way to offer customers to "Type" in ordering options.

For instance, after choosing Personalization: If yes is choosen, add $10 and allow them to type in the personaization?

Or are only drop down options available for each product?

I attached the paper form of an order form and I am trying to put into a JotForm.

Setting up the size and color is easy but I would like the customer to be able to TYPE in theor engraving choice and also offer the option of personaization for the extra $10

Thanks for reaching out to Jotform Support. What you can do is add an additional product choice to the product list and a field where they can enter their design and create a condition in which the field to enter their design will appear if that specific product is filled.
