I have a problem with certain fields not appearing in the submission report I receive. As well I have information showing up in incorrect fields - when I receive the submission on my end too.
Any help would be appreciated!

You must be referring to the Notification Email and Autoresponder Email on your jotform http://www.jotform.us/form/30511483649152
Please try adding a new Notification and Autoresponder email on your form to get the updated fields on the form reflect on the emails. You can use the Add New Email button to do this.
then check again the resulting submissions emails with the new Notifier and Autoresponder.
