Thanks for reaching out to Jotform Support. We offer free form templates that you can use online. Let me show you how to use the templates available.

1. Go to the My Forms page and click the Create Form button.
   
   
2. Choose the Use Template option in the middle of the screen.
   
   
3. Use the search bar to find a form template that best fits your needs. Once you find the form, click the Use Template button.
