(1) I upgraded, but my account does not show it. Here is the info from PayPal:

(2) I integrated with FB, app ID: 176217385757369, but it is not sending the info and so there is no submit button.

1. Your account is already premium, can you please clear your cache and then relogin?
2. It appears to be a problem with your iFrame Height especially when someone attempt to submit while some other required fields aren't filled which will increase up your form height. Can you increase your Iframe Height to 850px? Or you can re-embed your form using this code:
