My form is not working?

Hi, my form has stopped working on my site but is still working in the jot form preview, I have tried to re-embed it but still having the issue, please help!

I checked to see if I could also load "paperandlace.com" and your home page does not seem to load either.  It could be a problem with your servers.  Once your site is back up, let us know and we will be happy to check your embedded form.
