We have created a form using Microsoft Word and Adobe Acrobat X Pro. Is it possible to upload this form to JotForm to collect the responses?

I'm not sure if I am understanding you correctly or not but I believe you are asking if it's possible to import a form you created elsewhere in MS Word and Adobe Acrobat if that is correct?

To be honest I don't think it would work from MS Word but it might work from Acrobat.

First, you can try out this app. http://apps.jotform.com/app/spread2jot but if this doesn't work then I would suggest trying it with the following method below.

    1. Adobe FormsCentral allows your to import a PDF form. I would suggest to signup here so you can import your PDF form.

    2. Thus hopefully you might be able to create an account with Adobe FormsCentral before they close down since they will be retiring so soon. http://blogs.adobe.com/acrobat/farewell-formscentral/

    3. Once you have imported your PDF form into your Adobe FormsCentral account, you can use our import tool to import your Adobe FormsCentral form to Jotform. http://www.jotform.com/formscentral/

Alternatively, If you don't want to go that route we can try to help you create a similar jotform if you'd like.
