How do I delete a theme that continues to show up?

I used a theme from your theme categories and then decided to create my own.  I tried to delete the previous theme and upload the one I want but previous theme continues to show.  its weird because when I preview the form the correct one shows in the preview window while the other still exist.

I'm sorry for the trouble caused to you.

Upon checking your form, I found that it displays two different back ground image. When the form is loaded in the editor, it displays a different background but in preview it shows a different background image. Please check the screenshots below:
