Need a form to share with my customer

Asked by Tariq  on February 24, 2015 at 10:22 PM

    I specialize in human resources

    1. Looking for a form to pass it to my paid customer so that they answer my questions.

    2. Usually customer needs to more than a day to answer the questionnaire. Accordingly, the client wanted to be able to access and begin to answer and stop at any time and enter later to complete what he had done previously.

    3. Usually questionnaire requires upload some files and pictures by the client.

    4. The questionnaire will have some videos to explain some of the questions. I have them on YouTube clips. Can those movies be in the same form where the client can watch when he is answering the questions without going out.

5- what is the cost for your services.

6- I do have a website for my small office, and I am selling online my service, can I use the form that I will create with your service to be part of my site with my name,,,,
