    Hi,

    You can easily do this with JotForm, and the keyword is 'Lightbox'.

    Here are the typical steps:

    1. Finish your Form.

    2. In 'Setup and Embed' tab, choose 'Embed Form'.

    3. Choose 'Lightbox' amongst buttons.

    It will give you something like below:

    <script src="//www.jotform.com/min/g=feedback" type="text/javascript">

    new JotformFeedback({

    formId:'12953920476',

    base:'http://www.jotform.com/',

    windowTitle:'Contact Us Sample',

    background:'#FFA500',

    fontColor:'#FFFFFF',

    type:false,

    height:500,

    width:700

    });

    </script>

    <a class="lightbox-12953920476" style="cursor:pointer;color:blue;text-decoration:underline;">Contact Us Sample</a>

    --

    Since you need a button,

    Replace:

    <a class="lightbox-12953920476" style="cursor:pointer;color:blue;text-decoration:underline;">Contact Us Sample</a>

    with below:

    <input type="button" class="lightbox-theRightIdGoesHere" value="sample button" />

    or

    <input type="image" class="lightbox-theRightIdGoesHere" src="linkToImageGoesHere" />
