    Conditional Statements Still Not Working

    Please look at this form.  I believe I've modified the form questions conditional statements as provided.  Nevertheless, I just received a submission where the user answered No to each item, yet the Notice still shows.

    Clearly I'm not capable of fixing this.  Would you please lend a hand?

    There are four questions (yes/no) that are designed to show/hide a Notice.  Unfortunately, the Notice shows all the time, but should only show when any one of the four questions is answered "Yes."  I tried placing the condition that If Are you out of work due to your condition is equal to "No" then Hide Notice.  The problem is that if any one of the 4 questions response is "No" it doesn't matter if the other 3 are "Yes," the Notice is hidden. 

    So, what I get is either the Notice shows all the time or none of the time.  I only need it to show if any of the 4 questions are "Yes."  If all are "No" the notice should be hidden.  Need help.

    You can do this with a single Show condition. Please see the following animation:

    I tested and the conditions are working properly on the form:

    As the notice is a Text field, we only have the option of displaying Text fields and Headers or not in submissions:

    If you want them to display only if a condition is met maybe it is necessary to use another field type and apply some css so that it looks like a text field.
