    How do I create multiple entries from one form submission?

    Asked by lasalleyouth on February 26, 2015 at 03:00 PM

    Here is the situation:

    I need to create a registration form for a kid's program.  The parents will enter their information at the top, then enter their children's information (Child #1, #2, #3).  When the parents hit the submit button, I'd like three different entries to appear on the spreadsheet (one for each child, with the parent info connected to each child).

    I could force the parents to simply register each child separately (with a clean form each time), but that would require them entering their general family information each time (not cool).

    I did find a thread (http://www.jotform.com/answers/354790-Adding-multiple-entries-for-one-field) that talked about using a custom THANKYOU url with conditional logic that would pull the parent information onto a new form, allowing the parents to simply enter each child's info one at a time.  Each time I tried to use a custom THANKYOU url to pull the parent info I received error 405.

    Any help would be appreciated.

    Hi Steve,

    If you would like to have each child entered as its own entry, your user will need to fill our multiple forms. However as stated in the thread you mentioned, you can populate the parent information after they submit the form with their child's information.

    I have cloned your form and made changes to show how this can be achieved. You can view the form here:

    You stated that you had an issue with being redirected. After reviewing your form, I am not seeing the condition setup to redirect your users. It is possible that the URL was not entered correctly. This is the URL used on my sample form: http://form.jotformpro.com/form/50566144292960

    http://form.jotformpro.com/form/50566144292960?parentsName1={parentsName1}&manitobaHealth37={manitobaHealth37}&primaryPhone={primaryPhone}&cellPhone28={cellPhone28}&parentEmail={parentEmail}&address={address}&input30={input30}&input31={input31}&emergencyContact={emergencyContact}&emergencyContact39={emergencyContact39}

    It would be the same for your form, the difference being your forms URL.

    Please see the screenshot below for the two conditions added to the form:

    I hope this helps, please inform us if you need further assistance.
