Form hanging / not sending when used in the website

    My form seems to be not sending / hanging after users click the Submit button the final page. This is happening to multiple users on multiple computers/devices. It just says "Please wait..." but nothing happens. Please advise.

    It is highly possible that your jotform http://www.jotformpro.com/form/50605014966958 is having script conflict issue when embedded on your website due to the other existing scripts on the website already.

    May I suggest you remove the currently embedded form and then re-embed using its iframe embed code instead.
