I'm sorry for the trouble caused to you.
You see this error message when the uploaded file did not reach our server successfully. I did check your form's last several submissions (approx 40 submission records) you have received and found that in only one submissions the uploaded file does not get downloaded. I would suggest you to ask your users to re-upload the files again.
I am also reporting it to our back end team as well to check the possibility to recover it. We will get back to you as soon as we have any update from them.

I tried searching for the said file, but it could not be retrieved. As it appears, the file may have failed to complete its upload process promting the file name only to be captured and the file got cut off prematurely. This may have been caused by intermittent connections to the files being uploaded at the time of its submission.
As recommended by my colleague, ashwin. You will need to contact the person who uploaded the file and ask him to make another submission.
