As I understand, the images won't show up in the PDF submissions right? Please share the ID or URL of the form you are working with.
Go to submissions area. Under MyForms page, select the form and click the "Submission" button.
After that, click the "gear or properties" icon. Make sure that the upload field is enabled. Please try enable the "Show headers and text" in the submissions page as well.
What you see on the preview, is what you get when you download the PDF by clicking the "Get PDF" button at the top.
