Can you state which of the forms you are having this issue with so that we can do some checks?
You should also try to re-integrating the form with Google Sheet now to see if the issue remains. It was noted that Google had some issues error which code have cause this problem. Do let us know how it goes.

Darn, it's just still not working. I saw that Google was having some trouble earlier, but I just tried to integrate again and it says it's integrated, but the spreadsheet is not there and neither is the folder. It's a cloned form; I wonder if that has anything to do with it?

Can you click the link above and tell me if you receive an error message or could you indicate what happens after clicking the link.
Using a cloned form shouldn't cause this issue. I tested the integration with a cloned version of your form and I am still not able to replicate the issue.

Is the above mentioned form the only one with the issue? I will pass this information on to our development team so that they can do some checks. Please let us know if anything changes.

I can get to the spreadsheet using your link. I tried integrating it again, and I can get to it with the link. It just isn't showing up in my Google Drive.
I am renaming the spreadsheet and the folder it goes into. I don't know if that's the problem. I also tried it without renaming the folder and that didn't work either.

I checked my Google Docs account and I was able to locate both the folder and file for my integrated sheet.
I am not sure why this is happening in your account but I have passed this information on to our higher ups so that they can look into it. As soon as an update is available you will be notified via this thread.

Are you using a Google apps account for integration? If this is not the case, a recent correction may have fixed the issue. Otherwise, you may need to contact your Google App administrator to enable Drive API for your account.
