Dual Feedback Form is not display on my website

    could you please have look at this form some times its  working some times not.

    I was able to view the embedded Feedback Form http://www.jotform.co/form/42996841442868 on your website page.

    But the form is unusable when I tried using it on the website. The collapse fields are non-interactive so there is nothing to do on the form.

    Using the form URL http://www.jotform.co/form/42996841442868 work just fine.

    There must be a script conflict that is happening because there were 2 Feedback Forms embedded on the page.

    I suggest you test first by removing the other Feedback Form (Order Now) and test only using the Quote Request form. Check if the single form only will work without any conflict.
