When you view this form on an Iphone
most of the text input fields are VERY wide.
However all the other forms we use are normal and work fine
The only thing that is different is I used your "Designer" feature to change some features so I assume it is a setting that the Designer has implemented.

I was able to see the problem when I checked the form here
May I inquire though why you were using the full form source code embed of the form on the website?
Instead of just the default embed script or iframe embed script code.
Checking the source code of the 2 web page
I can see differences on the content of the source code.
I also noticed that you used multiple Mobile Responsive widget on the form http://www.jotformpro.com/form/41597954231966
Please try using only once, that should be enough for the form.
Actually using only the form's URL http://www.jotformpro.com/form/41597954231966 shows it as mobile responsive.
I will check further after I get more details as to why there is a need to use the full source code embed of the form on your website.
