Only the Top Portion of My Form Is Being Displayed on My Web Page. I'm Using the Full Source of the Form

Only the top portion of my form will display. I am using Everweb. 

In which browser are you experiencing this issue? In Chrome, although its styling is a bit different than at the form's own URL, the entire form is being displayed. (The difference in appearance is probably due to rules in the page overriding the form's rules.)
