My previous questions that I have asked on this forum have been deleted. This certainly seems as if Jotform is not at all interesting in combating phishing.

Take a look here at the question that was deleted:

Why is Jotform not interested in combating phising?

    Hi Foxp2,

Thank you for reporting those forms, we have greatly appreciated that. I would like to correct you that the thread wasn't deleted, my colleague set it to private so that other user's couldn't take advantage of the source code you gave. Furthermore, reported form/account was already deleted.

You can read below the full reply of my colleague Joey on that thread.

Thank you for reporting. We have suspended both the account and the form. They are disguising the form to show differently to us so they can beat our system. When we manually check these forms we know that it is a phishing form but it gets past our phishing filter. These type of forms are the hardest to detect for our phishing filter but thank you to you and our responsible users that we can avoid further damages.

I took this post to private because people may take advantage of the source you gave.
