Your forms were auto suspended because they are suspicious of phishing activities since you're requesting for sensitive information such as IMEI numbers, SIM Id, etc.

Can you explain the purpose of your forms? and why would you need such information?

You should be aware that we don't keep the submitted information in an encrypted way which means that if your account access gets compromised your information will also be compromised.
