Does jotform have the option for an approval form type

    At my school I have to approve request for leave, etc.  My staff fill out the form and then I sign to approve and send it back. 

     

    Also, your forms seems to have a top down process for the fields, can we make it look like the form like in Adobe where we have the form with the fields expanded from left to right to cover a letter page.
